Worksite, north - Employee engaged in work on a chimney was observed on a scaffold approximately 18 feet above the ground. Personnel had accessed the top platform by climbing the scaffold. Safe access was not available on, or about, 09/10/12. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
