29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) Pavilion, Southeast corner - Empoyees working directly under wooden soffit instalation did not have head protection; on or about 09/11/12.    Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.304(h): Each circular crosscut table saw was not guarded by a hood which shall meet all the requirements of paragraph (i)(1) of this section for hoods for circular ripsaws.    a) Pavillion, East Side - Employees were using an unguarded Skilsaw 3310, item# 006 table saw to cross cut wood for soffit installation; on or about 09/11/12.    Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    A) Pavillion, Southeast corner - Employees were using Lousivillle 6 foot folding A-frame ladders for soffit installation. The Ladders were leaning up against the side of the pavillion; on or about 09/11/12.    Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
