29 CFR 1926.403(i)(2)(iii): Entrances to rooms and other guarded locations containing exposed live parts operating at 600 volts, nominal, or less were not marked with conspicuous warning signs forbidding unqualified persons to enter: Bldg 23 & 24 Entrance to electrical room was not guarded or marked with a conspicuous warning sign forbidding entry to unqualified employees. Employees were exposed within a 10 foot arc flash boundary hazard because of a 480v/277, 100amp missing front panel.

29 CFR 1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means. Bldg 23 & 24: Employees working on energized 480v/3-phase/100amps electrical panel while troubleshooting were within the 10 foot ARC flash approach boundary. Employees were not provided with appropriate electrical protective equipment such as, but not limited to: voltage rated gloves, flame resistant clothing and arc rated face shield.
