29 CFR 1926.760(b)(3): The employer did not ensure that employees engaged in structural steel connecting work were provided, at heights over 15 ft, with a personal fall arrest system, positioning device system, or fall restraint system and wear the equipment necessary to be able to be tied off:    a)  Employees did not have a fall protection system available to tie off to when connecting a steel beam.  They were wearing fall protection harness and lanyards but did not have anchorage equipment to attach the lanyards to,  such as retractible lifelines, beam clamps, or choker cables. The beam became stuck requiring the two employees to position on the beam such that fall protection was needed.  The beam shifted suddenly and one employee fell 18 ft to the concrete floor.    Location:  WTC site, Tower 3, New York, N.Y.     On or about 8-29-2012     NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.     FALCON STEEL INC  WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR  ESSENTIALLY SIMILAR STANDARD,  29 CFR 1926.760(b)(3), WHICH WAS CONTAINED  IN OSHA INSPECTION NUMBER 312520315, CITATION NUMBER 1, ITEM NUMBER 1, ISSUED ON 2/27/2009, WITH FINAL ORDER DATE 3/27/2009.
