29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.    (a)  At the site, employees were exposed to a 12 foot fall hazard while working from a tubular welded frame scaffold, two frames high, that was not protected with guardrails at the platform ends.    (b)  At the site, employees were exposed to a fall hazard 12 feet high, in that, a single plank 10 inches wide was used to bridge from the scaffold to the roof in order to reach the exterior of the building during stucco operations.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a)  At the site, employees were exposed to a 13 foot fall hazard, in that, no fall protection was provided while standing on the roof of the building during stucco application.

Single plank and two plank platforms were used to reach the various work elevations.     (b) At the site, employees were working from a trestle scaffold platform that was not fully planked. A single 10 inch plank was used as a platform during stucco operations.
