29 CFR 1910.147(c)(6)(i):     The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.    On or about 09/11/2012, the employer had not conducted an annual review of the lockout/tagout  procedures for a equipment such as, but not limited to, Swaine Punch Presses, Bliss Punch Presses, Jordan Punch Presses, Rite Punch Presses, Rousselle Punch Presses, and Crimper Machines.   Most recent review was conducted 01/19/2010.

Documentation revealed evaluations for one employee was last conducted 11/24/1999.
