29 CFR 1926.602(d): The employer did not certify that each powered industrial truck operator had been trained and evaluated as required by 1910.178(l): (a) Construction Site - On or about September 12, 2012 an employee was observed operating a Lull 944E-42 forklift without first receiving training or an evaluation and certification, and was not wearing a seatbelt. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
