This condition potentially exposed employees to chemical hazards.

29 CFR 1910.1200(f)(5)(ii): The employer did not label each chemical container with the appropriate hazard warnings, or alternatively, words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: a) Nabors Completion & Production Services, 9540 E 104th Ave, Henderson, CO 80640: On and before September 10, 2012, the employer did not ensure that chemical containers were properly labeled. Two totes of Superior Well Services XL-4 2011221AC (1/2 full) and 2012006AC (full) crosslinker did not contain a hazard warning label containing the physical and health hazards of the hazardous chemical. This condition exposed employees to chemical hazards. *****instance b deleted***** b) Nabors Completion & Production Services, 9540 E 104th Ave, Henderson, CO 80640: On and before September 10, 2012, the employer did not ensure that chemical containers were properly labeled. A tote of 30% acetic acid was originally labelled as Clay Treat and then crossed out with black spray paint identifying the chemcial as 30% acetic acid. This container did not contain a hazard warning label identifying the physical and health hazards of acetic acid. This condition exposed employees to chemical hazards.

A label on a tote of LSG-2 CMHPG M-4.0 (full) was heavily damaged. This condition exposed employees to chemical hazards. (b) Nabors Completion & Production Services, 9540 E 104th Ave, Henderson, CO 80640: On and before September 10, 2012 the employer did not ensure that chemical labels were maintained in a legible condition. One tote of sodium bromate (partially full) contains a 1494 hazard warning label but the identification label is defaced or damaged. This condition exposed employees to chemical hazards. (c) Nabors Completion & Production Services, 9540 E 104th Ave, Henderson, CO 80640: On and before September 10, 2012 the employer did not ensure that chemical labels were maintained in a legible condition.The label on a tote of Clay Treat (full) is damaged. This condition exposed employees to chemical hazards. (d) Nabors Completion & Production Services, 9540 E 104th Ave, Henderson, CO 80640: On and before September 10, 2012 the employer did not ensure that chemical labels were maintained in a legible condition.The label on a tote of Paradene AW Hydraulic Fluid (full) is damaged. This condition exposed employees to chemical hazards. (e) Nabors Completion & Production Services, 9540 E 104th Ave, Henderson, CO 80640: On and before September 10, 2012 the employer did not ensure that chemical labels were maintained in a legible condition. The label on a tote of Motiva Star 2, Paraffinic Mineral Oil (full) is damaged. This condition exposed employees to chemical hazards.

An portable eyewash was being stored in the QC van however it was not available for use. This condition exposed employees to chemical hazards.
