29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length shall not extend over its support more than 18 inches (46 cm), unless it is designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end. a. On or about September 12, 2012 - at the above addressed jobsite, employees were performing siding installation from a ladder jack scaffold that had a platform extending greater than eighteen (>18") inches past its supports, thereby exposing the employees to a fall of approximately twenty five (25) feet to the ground.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a. On or about September 12, 2012, at the above addressed jobsite, employees performing siding panel installations from a pump jack scaffold, approximately twenty five (25) feet above the ground, were exposed to injuries from falls due to the lack of mid-rails and end-rails, thereby exposing the employees to falls.
