29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports. Employees engaged on installing an overhang with the use of a scaffold were exposed to fall hazards of approximately 6 feet from the working platform to the concrete below. The employees were exposed in that the working platform was not completely planked.

29 CFR 1926.452(a)(2): Crossbracing shall be installed between the inner and outer sets of poles on double pole scaffolds. Employees engaged on installing an overhang with the use of a scaffold were exposed to fall hazards of approximately 6 feet from the working platform to the concrete below The employees were exposed in that a cross bracing was missing permitting the scaffold to be unstable.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation. The employee using the scaffold did not have bases plates or mud sills to keep the scaffold from sliding.
