Kitchen Area/Food Warmer Machine - On or about September 13, 2012, employees were required to use electrical equipment that was not effectively closed. The electrical equipment is the following; 1. The outlet box was missing one knockout plug on its east side. 2. The outlet box was missing two knockout plugs on its west side.

Kitchen Area/Scale-Weighting Machine - On or about September 13, 2012, the electrical cord for the weigthing machine ( Model # LP-1000 and Serial # PK08103234) was missing its grounding pin. Employees were thereby exposed to electrical hazards.
