29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Workplace, iron and fold machines area of the facility- Employees perform service and maintenance functions on machines such as, but not limited to:  Lavatec Masterfold #3, model B112, Serial # B112R029561 and,  Lavatec Masterfold #4, model B112, Serial # B112R049559  There was no Lockout/Tagout program; on or about 09/07/12.    Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
