Lehigh County Detoxification Center, Roof -- On or about 31 August 2012, an employee was not wearing eye protection while using a gas powered nail gun to install roof sheeting, exposing employee to potential serious eye injury.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Lehigh County Detoxification Center, 6/12 Pitched Roof -- On or about 31 August 2012, two employees engaged in new construction/roofing work on the 6/12 pitched roof, one level commercial building. The two employees were not protected by guardrail systems, safety net system, or personal fall arrest system, exposing employees to a fall hazard of no less than 15 feet.
