29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about September 12, 2012 employees working from a steep roof with a 6:12 pitch were not protected from falling more than 40-feet to the lower level.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews. On or about September 12, 2012 employees were observed working on the second tier of a tubular welded frame scaffold with an incomplete guardrail. Employees applying stucco to the exterior of the building were not protected from falling more than 14-feet to the lower level. Stucco Texas was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(vii), which was contained in OSHA inspection number 314281205, citation number 01, item number 003 a, and was affirmed as a final order on November 3, 2011, with a final abatement date of October 19, 2011 with respect to a workplace located at 5301 Airport Freeway Fort Worth, TX 76117.
