29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: a) First Floor, Southwest Wall -- On or about September 7, 2012, an employee was doing demolition work on the first floor of a facility under renovations where there was a large window opening that was unguarded, exposing employees to a fall hazard no less than 8 feet.
