29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports. On or about September 12, 2012, employees were observed accessing the roof from a tubular welded frame scaffold that was not fully decked. Employees were exposed to fall hazards of approximately 13 feet to the concrete below.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On September 12, 2012, employees working from a steep roof with a 8:12 pitch were not protected from falling more than 9-feet to the lower level.
