29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a.) Baghouse - on or about 9/13/12 and prior, an employee had cleaned the baghouse filters, which is a confined space. the employer isolates the space prior to employee entery (breaking the plane), however, the employer had not trained the employee at an authorized employee level, so they could apply their own locks to the equipment, to ensure their personal safety. The employer did not train all employees who operate at an authorized level (equipment maintenance). ABATEMENT CONFIRMATION REQUIRED

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: (a) Baghouse - on or about 9/13/12 and prior, an employee was cleaning and working on the baghouse filters for a period of multiple days. the baghouse is a confined space, for which the employer has a written lockout procedure for electrical and pnuematic energies. Although the electrical and pnuematic were locked out by maintenance, the exposed employee did not lock out the energy sources. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a.)Facility- On or about 9/13/12 and prior, employees work with large amounts of cement, that contains silica. The cement is is powder form, prior to the addition of water and additives, and after drying can be made into respirable dust during grinding, hammering and clean-up operations. Employees can be exposed to silica-containing dust during a variety of different operations. The employer has not provided the employees, who hae potential exposure to silica-containing dust with Hazard Communication training that provides inforamtion on the hazards, and how employees can portect themselves. ABATEMENT CERTIFICATION REQUIRED
