29 CFR 1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation and the identity of the person performing the training or evaluation: All operator training and evaluation shall be conducted by persons who have the knowledge, training, and experience to train powered industrial truck operators and evaluate their competence. a) Warehouse/Production Area: The employer failed to certify that the forklift operators were trained and evaluated to operate industrial forklifts. Armal, Inc. was previously issued a citation for violation of the OCCUPATIONAL SAFETY AND HEALTH standard 1910.178 (l)(6), Citation 2, Item 3. OSHA inspection # 316079722. Final Order was March 12, 2012.
