Water Plant Well House. A 480 volt Square D motor starter electrical panel, was not labeled to inform employees of electrical arc hazards to which they may be exposed, as observed on January 23, 2013.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards were not provided with, and/or did not use, electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed:    a. Martinsburg VA Medical Center: Employees work on live electrical parts while troubleshooting energized equipment such as, but not limited to, the motor starter at the Water Plant Well House. Employees are exposed to 480 volts, and are not wearing appropriate electrical protective equipment such as, but not limited to, voltage rated gloves and flame resistant clothing, as determined on or about January 23, 2013.  b. Martinsburg VA Medical Center: Employee(s) were exposed to live electrical part while obtaining the serial # and model # from inside the transformer located near the Heating Plant. Employee(s) were exposed to 480 volts and was not wearing appropriate electrical protective equipment such as, but not limited to, voltage rated gloves and flame resistant clothing, as determined on or about January 23, 2013.    MARTINSBURG VA MEDICAL CENTER WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1910.335(A)(1)(I), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311680862, CITATION NUMBER 1 ITEM NUMBER 5, ISSUED ON AUGUST 4, 2010, AND ABATEMENT WAS CERTIFIED ON SEPTEMBER 30, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 510 BUTLER AVENUE, MARTINSBURG, WV.
