At the site, employees were installing new gutters and soffit on a residential home from a tubular scaffold that was not positioned on base plates or mud sills for a solid foundation, thereby exposing the employees to a fall hazard of approximately 15 feet.

A) At the site, employees were installing new gutters and soffit on a residential home from a tubular scaffold and a ladder jack scaffold. One end of the ladder jack scaffold platform was resting on top of the tubular scaffold planking, bridging one platform to another, thereby exposing the employees to a fall hazard of approximately 15 feet.
