29 CFR 1926.404(b)(1)(ii): All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which are not a part of the permanent wiring of the building or structure and which are in use by employees, did not have approved ground-fault circuit interrupters for personnel protection. a) On August 22, 2012; second floor, employees were operating electrical tools such as but not limited to skill saws which were not plugged into a ground fault circuit interrupter.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. a) On or about August 22, 2012; front roof area; employees conducting roof work on a 6 in 12 pitch roof were not protected by a personal fall arrest system to prevent them from falling 18-20 feet to ground level. b) On or about August 22, 2012; second floor; wall openings were not guarded with a top rail to protect employees from falling 12 feet to ground level.
