(b) Shop: On or about 13 September 2012 and at times prior, the employer had not developed a written, machine specific procedure to include specific procedural steps for shutting down and isolating all potential energy sources, including but not limited to the electrical (440) and hydraulics, for the Cincinnati 300 injection molding machines prior to the commencement of machine repair and maintenance operations.
