29 CFR 1926.501(b)(2)(i):  Employees constructing a leading edge 6 feet (1.8 meters) or more above lower levels were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:          On or about 9/13/12, an employee was exposed to the hazard of falls from an elevation of approximately 12 ft. to the ground level below while installing a roof decking, without fall protection.

29 CFR 1926.503(a)(2)(iii):  The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:             On or about 09/13/2012, at the worksite, employees that were not trained in fall protection were exposed to a fall hazard from an elevation of approximately 12 feet in height to the ground level below while installing a roof.
