29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: On or about September 6, 2012, on the west side of the construction site, a pile of debris had accumulated near the entrance to the jobsite, exposing employees to the hazards of lacerations and/or puncture injuries caused by stepping on nails.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. On or about September 6, 2012, employees working from a steep roof with a 10:12 pitch were not protected from fall hazards of more than 25-feet to the lower level.

On or about September 6, 2012, employees climbed 12-feet from the lower surface level to access the upper working surface level without the use of ladder, exposing employees to fall hazards.
