(a)  The Cooler Storage room used to store approximately 191 gallons of Category 3 flammable liquids with a flashpoint below 100 F and approximately 180 gallons of Category 2 flammable liquids, was not equipped with wiring approved for Class I, Division 2 Hazardous Locations.     (b)  The Sprinklered Rack Storage room used to store approximately 674 gallons of Category 3 flammable liquids with a flashpoint below 100 F and approximately 424 gallons of Category 2 flammable liquids,  was not equipped with wiring approved for Class I, Division 2 Hazardous Locations. 29 CFR 1910.106(e)(2)(iv)(d):  Flammable or combustible liquids shall be drawn from or transferred into vessels, containers, or portable tanks within a building only through a closed piping system, from safety cans, by means of a device drawing through the top, or from a container or portable tanks by gravity through an approved self-closing valve. Transferring by means of air pressure on the container or portable tanks shall be prohibited:    (a) On or about November 6, 2012, Category 2 flammable liquids were not transferred between containers through a closed piping system, in that diacetyl was transferred from a 55 gallon drum to an open metal container in the compounding area. 29 CFR 1910.134(d)(3)(i)(B)(1): The employer must select a respirator for employee use that maintains the employee's exposure to the hazardous substance, when measured outside the respirator, at or below the Maximum Use Concentration (MUC).    (a) The employer did not select a respirator that maintained the Compounder's exposure to diacetyl at or below the MUC based on the American Conference of Industrial Hygienists (ACGIH) Threshold Value (TLV) of 20 Parts Per Billion (PPB) as a 15-minute Short Term Exposure Limit (STEL). Specifically, the Compounder was exposed to diacetyl at 1,800 PPB as a 15-Minute Time Weighted Average during a sampling period of 50 minutes, which exceeded the MUC for a full-face respirator of 1,000 PPB based upon a protection factor of 50.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
