29 CFR 1926.451(f)(15)(iv): The ladder legs shall be secured to prevent them from slipping or being pushed off the platform.      a) 171 I-310 Service Rd., St. Rose, LA; The employer failed to ensure that a ladder used to access a scaffold was on stable surface. The ground was uneven mud and rock and the ladder was on two pallets and a wooden board. The ladder extended approximately 16 feet above the ground. This condition exposed employees to a fall hazard.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold shall be protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity).      a) 171 I-310 Service Rd., St. Rose, LA; Employees were exposed to a fall from elevation while working from a scaffold that did not have a complete guardrail system and the employees were not protected by personal fall arrest systems. Employees were exposed to an approximately 18 feet fall.
