29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph. a) Jobsite: 135 Ellsworth St., Philadelphia, PA. - On or about 9/11/12, the supported tubular welded frame scaffold, measuring approximately 40 feet in elevation and 5 feet in width, was not restrained from tipping or falling over by guying, tying, bracing, or equivalent means. Employees were performing masonry operations from the outrigger section on the 4th working platform, approximately 26 feet above ground level.
