Location: Jobsite, 53 Fairview Street a) Employee doing exterior upgrades from a ladder jack scaffold was trained to recognize and avoid hazards associated with using the scaffold. Employee was working on the scaffold without fall protection, thus exposing employee to falling up to 20 feet to the ground below on or about 09/08/12.
