On or about Friday, September 14, 2012 at 99-25 Horace Harding Expressing, Corona, NY 11368    Employees were observing using propane torches to secure roofing tiles to a commerical rooftop without eye and face protection exposing the employees to eye and/or face injuries.

A.) On or about Friday, September 14, 2012 at 99-25 Horace Harding Expressway, Corona, NY 11368    Two (2) employees were observed working on a commerical rooftop approximately 20 feet above the lower level installing roof material without a means of fall protection in place exposing the employees to fall hazards.
