29 CFR 1926.451(g)(1): Each employee on a scaffold 10 feet above a lower level was not protected from falling to the lower level: (a) B&H General Contractors, Inc. at 2014 Grand Ave., Glenwood Springs, CO 81601: On and before 9/12/12, the controlling employer did not ensure that employees were protected from a fall from elevation in that guardrails or other suitable fall protection were not properly installed on the working platform of the scaffolding. This condition exposed employees to a fall of approximately 15 feet. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet"). Abatement Note: The employees were not protected by guardrails installed in accordance with 29 CFR 1926.451(g)(1). The scaffold was fully braced however, the bracing did not meet the requirements as a partial guardrail as detailed in 29 CFR 1926.451(g)(4)(xv).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) B&H General Contractors, Inc. at 2104 Grand Ave., Glenwood Springs, CO 81601: On and before 9/12/12, the controlling employer did not ensure that each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system. Employees were plastering the west side and south side of the building located at a parapet measured at 1'-10 inches and 2 feet wide. This condition exposed the employees to a fall hazard of approximately 21 feet. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
