29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed: (a) The employer did not ensure that all affected employees were removed from the hazardous area prior to testing equipment, such as by conducting a visual inspection of the area to identify personnel. On or about 8/17/2012 during maintenance/servicing on the Hall Machine #24, affected employee(s) were not informed of the actuation of the Hall Molding Machine's cylinders, exposing employee(s) to crushing and/or amputation hazards.

DBA DMI COLUMBUS LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD [29 CFR 1910.147(c)(6)(i)], WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311404628, CITATION NUMBER 1, ITEM NUMBER 5B AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 22, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 1600 NORTHSIDE INDUSTRIAL BLVD., COLUMBUS, GEORGIA.
