29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: On or about 09/07/12, employees were removing core drill stems in an excavation with vertical walls that was approximately 13 feet deep, 30 feet wide and 77 feet in length, in Class B soil with an approximate 2x2 foot piece of loose soil approximately 6 feet above the employees exposing the employees to struck by hazards.
