(b) Warehouse: On or about 05SEP12, 0949hrs, the latching hasp for the door was missing with the opening left uncovered on the circuit breaker panel labeled Panel 1 on the inside. (c) Warehouse: On or about 05SEP12, 0949hrs, black tape was used to hold a plastic covering over position 36 on the circuit breaker panel labeled Panel 1 on the inside.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Warehouse: On or about 05SEP12, 0922hrs, exposed conductors were not provided protection from a pull being directly transmitted to joints or terminal screws where the sheathing of a flexible cord was pulled back from the connection plug on the Hawker Powersource Lifeguard charger next to the circuit break panel.
