29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)      a) Jobsite, 4 Colonial Place, Pittsburgh, PA:  On or about September 17, 2012, employees were working on a scaffold platform that was not fully planked while installing fascia board onto the front of the residential house they were re-roofing.  The fall hazard was approximately 20 feet to the ground below.

29 CFR 1926.451(b)(10):  Scaffold components manufactured by different manufacturers were intermixed such that the components fit together with force and the scaffold's structural integrity was not maintained by the user:    (a) Jobsite at 4 Colonial Place, Pittsburgh, PA:  On or about September 17, 2012, a scaffold was used by employees to install fascia board. The scaffold platform was supported by a 24 foot aluminum ladder and fabricated frame stair tower which are not compatible components.

29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    a) Jobsite, 4 Colonial Place, Pittsburgh, PA:  On or about September 17, 2012, employees were not provided a safe means of access and egress to and from the stairtower scaffold and to the upper, main roof to perform roofing activities.  The distance from the scaffold platform to the roof was approximately 3 feet and the fall hazard was approximatley 20 feet to the ground below.

29 CFR 1926.451(f)(6):  Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):    a) Jobsite, 4 Colonial Place, Pittsburgh, PA:  On or about September 17, 2012, employees erected and dismantled a pumpjack scaffold approximately 3 feet from the 240 volt electric service power lines.  Employees engaged in the scaffold erection and dismantling were exposed to the hazard of electric shock.

29 CFR 1926.451(g)(1)(vii):  Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    a) Jobsite, 4 Colonial Place, Pittsburgh, PA:  On or about September 17, 2012, a stairtower scaffold used by employees to access the roof was not equipped with guardrails and the employees that used this were not protected from falling by any other means.  The fall hazard to the employees was approximately 20 feet to the ground below.

29 CFR 1926.451(g)(2): The employer did not provide fall protection for employees erecting or dismantling supported scaffolds:    a) Jobsite, 4 Colonial Place, Pittsburgh, PA:  On or about September 17, 2012, an employee was in the process of moving a scaffold pick that he had been working from on a flat section of the roof on a two story residential dwelling.  The employee was protected from falling during the dismantling.  The employee fell approximately 23 feet to the ground below sustaining fatal injuries.    OR IN THE ALTERNATIVE    29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:    a) Jobsite, 4 Colonial Place, Pittsburgh, PA:  On or about September 17, 2012, an employee was working from a flat section of roof on a two story residential dwelling was not protected from falling.  The employee was installing fascia board on the upper main roof.  The employee was exposed to a fall hazard of approximately 23 feet to the ground below.

Geocel 2300 roof sealant and Geocell 2321 gutter sealant) that the employees work with.
