29 CFR 1904.39(a): Within eight (8) hours after the death of any employee from a work-related incident or the inpatient hospitalization of three or more employees as a result of a work-related incident, the employer did not orally report the fatality/multiple hospitalization by telephone or in person to the Area Office of the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. (a) Growers Chemical Corporation: On or about September 12, 2012, the employer did not report, within the required eight hours, an employee fatality that occurred at work. The employer reported the fatality to OSHA Toledo Area Office on September 14, 2012.
