29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    a) KLB Industries, Inc. dba National Extrusion & Manufacturing - Bellefontaine, OH: On or about September, 17, 2012, employees were exposed to a fall hazard of approximately 71-inches when accessing the top of a cabinet to check oil levels of the extruder.  Employees were not protected by a standard guardrail, or equivalent protection.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):    a) KLB Industries, Inc. dba National Extrusion & Manufacturing - Bellefontaine, OH: On or about September 17, 2012, the employer did not assure that the Yale forklift (s/n 408215) was removed from service during a time in which it was in need of repair.  Employees were exposed to injuries related to unsafe operating conditions in that the clamps that secure propane tank on the truck were defective.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.217(f)(2): Supervision to ensure that correct operating procedures were being followed on mechanical power press(es) was inadequate:    a) KLB Industries, Inc. dba National Extrusion & Manufacturing - Bellefontaine, OH: On or about September 17, 2012, the employer did not assure that the operator(s) of the new Rousselle No.6B power press was instructed in the safe operation of the press in that the selector switch for the press run mode was not supervised.  The key, used to change press modes, was kept in the selector switch and not supervised to assure that the press mode was not changed without the operators knowledge.    b) KLB Industries, Inc. dba National Extrusion & Manufacturing - Bellefontaine, OH: On or about September 17, 2012, the employer did not assure that the operator(s) of the old Rousselle power press #29 (s/n FWSA 7451) was instructed in the safe operation of the press in that the selector switch for the press run mode was not supervised.  The key, used to change press modes, was kept in the selector switch and not supervised to assure that the press mode was not changed without the operators knowledge.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
