29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Warehouse - On or about September 17, 2012, a maintenance employee filled the electric powered industrial truck batteries (containing sulfuric acid) with water. The nearby eyewash did not have adequate flow for both eyes.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: a) Dry Goods Warehouse near Conference Room - On or about September 17, 2012, employees used a trash compactor that did not have adequate guarding. The compactor door can be opened easily by operator while compactor is operational.
