29 CFR 1926.451(g)(3)(iv):     Vertical lifelines, independent support lines, and suspension ropes were attached to each other, attached to or used the same point of anchorage, or they were attached to the same point on the scaffold or personal fall arrest system.    a) Jobsite: 604 South Washington Square  A vertical lifeline being used as a part of a personal fall arrest system was attached to the same anchorage point of a two point suspended scaffold. Two employees were performing restoration operations while on a suspended scaffold approximately 200 feet above ground level while they were exposed to these conditions. Observed on or about 07/30/12.
