The employer did not ensure that employees utilized 100% fall protection as required by the standard.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder(s) was/were not secured at its top to a rigid support, and/or a grasping device, such as a grab rail, was not provided to assist the employees in mounting and dismounting the ladder: 808 S 11th Street, Norfolk, NE 68701 (jobsite): Employees were observed installing siding on a stair/tower scaffold. The employees were observed using a portable extension ladder to access the working surface of the scaffold. The ladder did not extend 3 above the upper landing surface, nor did it meet any of the requirements of this subparagraph.
