29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about September 14, 2012 an employee servicing and overhead garage door was exposed to the hazards associated with the unexpected startup as the door opener was not de-energized and locked out. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
