(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY)      (a)	  Roof of the De-Nitrogen Building - There where three holes that where twelve and one half inches by thirteen inches by nine and one-half inches deep with no covers. These holes were for the water pipes as they exited the roof membrane, on or about September 14, 2012.            Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
