This condition occurred on or about September 11, 2012, in the Laser Building, where an employee was observed bending metal parts at the point of operation on a Bystronic Beyeler Xpert 200 press brake (S/N 854011) without the use of hand-feeding tools and where laser barrier guards had been disconnected.

This condition was observed on or about September 11, 2012, in the E-Shop where the pathway to electrical panelboards/circuit breakers was blocked by wood pallets, boxes containing metal parts and tools, and an unused welding machine.
