29 CFR 1926.252(a):     An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):    a) On or about 9/13/12 at the Kronenberg Building rehabilitation project located at 12 Main Street, Hamburg, New York; employees clearing debris off from the roof were throwing it over the side to a dumpster approximately 28 feet below. There was no chute or equivalent in place.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(4)(i):     Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:     a) On or about 9/13/12 at the Kronenberg Building rehabilitation project located at 12 Main Street, Hamburg, New York; employees working on the roof were placing materials, clearing debris and repairing the roof decking. Employees were not protected from falling through the ladder hole, or the holes in the roof deck, 14 feet to the floor below.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:     a) On or about 9/13/12 at the Kronenberg Building rehabilitation project located at 12 Main Street, Hamburg, New York; employees were repairing the roof deck and sheathing on a 65'x65' flat roof. There was no fall protectiion system in place and employees were exposed to falling approximately 28 feet.    NO ABATEMENT CERTIFICATION REQUIRED
