29 CFR 1910.1030(h)(1)(ii)(B): The medical record for an employee with occupational exposure did not include a copy of the employee's hepatitis B vaccination status, including the dates of all the hepatitis B vaccinations or any medical records relative to the employee's ability to receive vacination as required by 29 CFR 1910.1030(f)(2): a) Establishment - Hepatitis B vaccination records were not maintained for all potentially exposed employees who opted to receive the vaccination series. Approximately seven employees potentially exposed to blood and other potentially infectious materials (OPIM), and who had not originally declined the vaccination series, failed to receive the vaccination series in the required time frame. On or about 9/18/12. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

There was one unrecorded needle stick that occurred on 4/11/11. On or about 9/18/12. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
