29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): a) 3rd. Street, NE - Two utility workers installing an electrical duct bank in an excavation, comprised of Class B soil and measuring 5 feet wide, 30 feet long, and 7.5 feet deep, was not provided with shoring in the South end of the excavation and the remainder of the excavation was provided with inadequate wood shoring that did not meet the requirements of Appendix C, Table C-2.2, on or about 09/17/2012.
