Bollegraaf conveyor area - On or about September 17, 2012, a procedure with an equivalent level of protection to a personal lockout device was not developed for a crew of employees who perform ground level conveyor cleaning operations.  Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.
