29 CFR 1910.107(b)(5)(i):     The spraying operations except electrostatic spraying operations shall be so designed, installed and maintained that the average air velocity over the open face of the booth (or booth cross section during spraying operations) shall be not less than 100 linear feet per minute. Electrostatic spraying operations may be conducted with an air velocity over the open face of the booth of not less than 60 linear feet per minute, or more, depending on the volume of the finishing material being applied and its flammability and explosion characteristics. Visible gauges or audible alarm or pressure activated devices shall be installed to indicate or insure that the required air velocity is maintained. Filter rolls shall be inspected to insure proper replacement of filter media.    On or about September 13, 2012 and times prior thereto, an employee of Kitchen and Bath Ideas, Inc. performed spray operations with the use of flammable chemicals such as but not limited to high build lacquer sealer, high build semi-gloss, and solid color oil stains in the spray booth located at 3518 Pointer Trail East in Van Buren, Arkansas.  The ventilation system installed on the spray booth had an average air velocity of 15 feet per minute (fpm) over the open face of the booth, exposing employees to the potential for overexposure of chemicals and fire hazards.

29 CFR 1910.107(b)(10):     When spraying areas are illuminated through glass panels or other transparent materials, only fixed lighting units shall be used as a source of illumination. Panels shall effectively isolate the spraying area from the area in which the lighting unit is located, and shall be of a noncombustible material of such a nature or so protected that breakage will be unlikely. Panels shall be so arranged that normal accumulations of residue on the exposed surface of the panel will not be raised to a dangerous temperature by radiation or conduction from the source of illumination.    On or about September 13, 2012 and times prior thereto, employees of Kitchen and Bath Ideas, Inc. performed spray operations on wood cabinets with the use of chemicals such as but not limited to high build lacquer sealer, high build semi-gloss, and solid color oil stains in the spray booth located at 3518 Pointer Trail East in Van Buren, Arkansas.  Light fixtures had been installed on the left, right, and top of the spray booth for illumination.  The light fixtures did not have panels installed to effectively isolate the spraying area from the bulbs or to protect from flammable residue accumulation, exposing employees to fire hazards.

The spray booth and work area inside of the paint shop surrounding the booth had an accumulation of flammable residue, exposing employees to fire hazards.

A respirator previously used by an employee had been observed being stored on a work table and was not protected from damage, contamination, dust, or damaging chemicals and was not packed or stored to prevent deformation of the facepiece and exhalation valve.
