29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b).    Worksite:  Employees who were performing roofing work on a steep roof were exposed to fall hazards of approximately 22 feet.    TCHE Bagual LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313515892, citation number 2, item number 4 and was affirmed as a final order on 10/06/2009, with respect to a workplace located at Lot # 34 Hidden Brook Road in Bethel, CT.    THCE Bagual, LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 310185129, citation number 1, item number 5, issued on 8/15/2007 with respect to a worksite located at 2340 Sullivan Ave in South Windsor, CT and became a final order on 9/19/2009.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Worksite:  The employer did not ensure that all employees had been adequately trained in fall hazards through a documented training program which covered the hazards associated with falling and the proper ways and procedures to follow in order to minimize these hazards.    TCHE Bagual LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 313515892, citation number 1, item number 4b and was affirmed as a final order on 10/06/2009, with respect to a workplace located at Lot # 34 Hidden Brook Road in Bethel, CT.
