29 CFR 1910.36(b)(1) At least two exit routes were not available in the workplace to permit prompt evacuation of employees and other building occupants during an emergency, nor was there a second exit rout located as far away as practical from each other so that if one exit route is blocked by fire or smoke, employees can evacuate using the second exit route.    On September 18, 2012, the building that houses Advanced Paperworks only had one emergency exit for seventeen employees, which also served as the front entrance door.  No other emergency exit door was made available to employees so that if one exit route was blocked, employees would be able to use a second emergency exit to evacuate safely.

29 CFR 1910.36(c)(1)  Each exit discharge did not lead directly outside or to a street, walkway, refuge area, public way, or open space with access to the outside.    On September 18, 2012, the building that houses Advanced Paperworks only had one emergency exit for seventeen employees, which also served as the front entrance door.  The employer had designated a rolling warehouse dock door as an emergency exit but the discharge leads to train tracks.

29 CFR 1910.212(a)(1)  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    On September 18, 2012, two Mark Andy 630 and one Mark Andy 830 presses were not equipped with proper machine guarding to protect the operator from point of operation hazards, ingoing nip points from rollers, and rotating parts.  Employees work in a close proximity to the moving parts of the presses, sometimes leaning up against moving parts, to perform job functions.  A metal guard had once been equipped with the presses but is no longer available.

29 CFR 1910.305(g)(1)(iv)(A) Flexible cords and cables were used as a substitute for the fixed wiring of a structure.    On September 18, 2012, machinery for two Mark Andy 630 presses and one Mark Andy 839 press were being powered by the use of flexible cords.  In addition, employees were using fluorescent lighting, local air conditioning ventilation, and computer equipment in the facility which were wired and powered by the use of flexible cords instead of through fixed wiring of the building.
