29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: Employees were exposed to the hazard of electrical shock when working on the elevator bin deck near the southeast corner of the head house because the flexible power cord providing power to the elevator fan was not connected to the disconnect box with devices and fittings that would have prevented tension from being transmitted to the cord.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Employees were exposed to fall hazards when working from work platforms in the following instances: a) The bin deck of bin 19 had a fall hazard of approximately 100 feet to the ground below because the area adjacent to the transfer conveyor was not protected by a standard railing. b) The loadout platforms of bins 14, 15, 16, 19, 20, and 21 had fall hazards of approximately 5 feet to the ground below because one side did not have a standard railing.
